On your first visit to our office, our receptionist will ask you
to complete a patient information form. The questions we ask will
provide the doctors with essential data on your health and medical
history. You will also be asked questions concerning your insurance
and billing information. There will be times when we will have to
contact you, so please provide as complete information as possible.
Periodically, you will be asked to update your patient file in order
to receive the best care and service.
