Prepare for Your Visit

On your first visit to our office, our receptionist will ask you to complete a few patient information forms, or for your convenience, you may fill out these forms prior to your arrival to our offices. Each patient should fill out the patient information for and either the Adult evaluation or Pediatric evaluation whichever is appropriate for the patient.

Please be sure to bring the completed forms to your appointment.

The questions we ask provide the doctors with essential data on your health and medical history. You will be asked questions concerning your insurance and billing information. There will be times when we may have to contact you, so please provide as complete information as possible. Periodically, you will be asked to update your patient file in order to receive the best care and service.
