Patient Appointments
We know how important it is for you to get your appointment scheduled promptly. To request an appointment online please click here, or the button below. Our office will contact you within 24 hours or the following business day to schedule your appointment.
If you are an established patient of Cardiology Associates, you may also request an appointment online via secure messaging through our Patient Portal. In the portal, you can also request for prescription refills, submit questions and provide updated information.
You can also call our office to schedule at 251-607-9797 Monday through Friday from 8:00 a.m. to 5:00 p.m.
REFER A Patient
If you would like to submit online patient referral information please click here, or the button below and we will contact your patient within 24 hours or the next business day to schedule the requested appointment.
Also, please indicate on the form if you would like the patient’s appointment details for your records and we will contact your office with that information.
For same-day appointment requests, please call our office at 251-639-0202.
We will send by fax or Direct ID the patient’s appointment follow up information or test results to your office.
No Show Policy
We require at least 24 hours’ notice to reschedule or cancel an appointment. Missed appointments or cancellations made without enough notice will result in a $50 cancellation fee. Please contact our office as soon as possible to avoid this charge. If you miss an appointment for any reason, please do not come into the office without first rescheduling another appointment.
