How to Sign Up
To access your Cardiology Associates medical records online, click the Sign Up button or scan the QR code below.
You may also call our office at 251-607-9797 to request an invitation to create your FollowMyHealth patient portal account.
Cardiology Associates Patient Portal
Access your medical records anytime—24 hours a day, 7 days a week—using a computer, smartphone, or tablet through the FollowMyHealth® mobile app or the website at cardassoc.followmyhealth.com.
A patient portal is a secure online tool that gives you easy access to your personal health information and helps you take an active role in managing your care. FollowMyHealth® by Allscripts is the patient portal used by Cardiology Associates.

Accessing Your Medical Records in FollowMyHealth
With a FollowMyHealth patient portal account, you can:
See your medical records and test results
Ask for prescription refills
Send and receive secure messages with your Cardiology Associates care team
Connect with other doctor offices that also use FollowMyHealth

About test results and images:
You will be able to see the written results from tests like an echocardiogram, CTA, or cardiac PET in your portal. The moving images from these tests cannot be viewed online.
If you need the images:
You must complete a medical records release form (Click here)
Images can be sent to your referring doctor at no cost, or
Images can be provided to you on a CD or USB drive for a $20 fee
