Dr. David Mayer and Kathy Giattina, RN, MSN retired from Pediatric Cardiology on June 30, 2022. They will be missed at Cardiology Associates and the entire pediatric cardiology team will miss all of the patients they were honored to serve.

If you were a patient or your child was a patient of Dr. Mayer’s, a notification was mailed in May with information to help transition heart care to another provider. For additional reference, included below is a list of pediatric cardiologists and adult congenital heart programs in and around our area and also information to access medical record information.

If you are patient who is 17 or older it may be appropriate for you to transfer your heart care to one of our adult cardiologists. If you are an adult patient with complex congenital heart disease, it is advised for you to follow up with an adult congenital heart specialist in conjunction with an adult cardiologist. If you are uncertain of the appropriate care path, one of our adult cardiologists can evaluate and advise you on a care plan. Please contact our office at 251-607-9797 to schedule an appointment for this evaluation.

Accessing your medical record information can be done through a FollowMyHealth patient portal account, or by submitting a request to our office to receive a digital or printed copy. Additional information and details can be found at the following link: https://www.cardassoc.com/office-policies under medical records.

Please know that Cardiology Associates will continue to retain medical record information so that they are available should you need them in the future.
